1. Centre Registration Form(Downloadable below). (File size should be between 50Kb – 500Kb and in jpg, jpeg or png format)
2. Centre In-charge / Principal aadhaar card. (File size should be between 10Kb – 100Kb and in jpg, jpeg or png format)
3. Cancelled cheque (not mandatory). (File size should be between 50Kb – 500Kb and in jpg, jpeg or png format)
